- Birth Certificate
- Government Issued ID with Photo
- Marriage Certificate, if applicable
- Valid and Updated NBI Clearance
- Pre-employment Medical Exam
- Pre-employment Drug test
- School Document
- Proof of Employment
Critical Government Requirements
- Proof of SSS Number
- Proof of Philhealth Number
- Proof of Pag-ibig Number
- Proof of BIR Number/ Tax Identification Number (TIN)
Government Form
- Accomplished BIR 1902 Form (This is only applicable to employees who do not have TIN yet. This should be submitted to HR Benefits.)
Other Document/s
- BIR Form 2316 of the current year (This is applicable to employees who worked for another company within the year.)
